Birth Certificates & Death Certificates


    Individuals born in the State of Louisiana may obtain a birth certificate or family members for those  whose death occurred in the State of Louisiana may obtain a death certificate from the Donaldsonville or Gonzales Offices.  Birth and Death Certificates are issued Monday through Friday (excluding holidays) between the hours of 8:30 am and 3:30 pm.  Birth Certificates are available for persons born 1903 through the present. Birth Certificates for persons born prior to 1983 may have to be updated by the Vital Records Office. This process may take 30 minutes to one hour to update and print.  Death Certificates are available from 2013 through the present.  Birth Certificates and Death Certificates are printed directly from a Vital Records link and cannot be altered or amended by the Clerk of Court’s Office.  

     For all out of state birth and death certificates, you must contact that states Vital Record’s office.

    If you need a death certificate before 2013, you must contact the Vital Record’s office in New Orleans, LA.  Those who seek to alter or amend a birth/death certificate must also do so through the vital record’s office in New Orleans, LA.  Telephone: 504-593-5100

  • Who May Apply For A Birth Certificate: Person listed on the certificate or An immediate family member, such as
    • -       Mother

      -       Father

      -       Current legal spouse

      -       Child

      -       Sibling

      -       Maternal grandparent

      -       Paternal grandparent (if the father is listed on the certificate)

      -       Legal Guardian (Legal Guardian must have a certified copy of a judgment of custody that has been SIGNED BY A JUDGE. Custody by mandate and provisional custody signed by a notary are not accepted.  Transfer of custody witnessed by a notary cannot be accepted.

    •  Step-parents, step-grandparents, step-children, aunts, uncles, cousins, in-laws, ex-spouses, CANNOT obtain a birth certificate


    • Who May Apply For A Death Certificate: An immediate family member, such as
    • -       Mother

      -       Father

      -       Current legal spouse

      -       Child

             -        Sibling

      -       Maternal grandparent

      -       Paternal grandparent (if the father is listed on the certificate)

      -       Legal Guardian (Legal Guardian must have a certified copy of a judgment of custody that has been SIGNED BY A JUDGE. Custody by mandate and provisional custody signed by a notary are not accepted.  Transfer of custody witnessed by a notary cannot be accepted.

      -      Beneficiaries may request a death certificate, but MUST have a copy of the Insurance policy showing the applicant’s name listed as beneficiary or provide a copy of the Last Will and Testament naming the applicant as beneficiary.

      -      Funeral Directors may request additional copies of a Death Certificate on behalf of the surviving immediate family members within one year of the date of death.


      Step-parents, step-grandparents, step-children, aunts, uncles, cousins, in-laws, ex-spouses, CANNOT obtain a death certificate unless they fall in one of the above categories.


    • Identification Requirements: Applicants must present one form of identification from the list below:
    • -         A current state issued driver’s license

      -         A current state issued identification card

      -         A current U.S. Passport

      -         A current U.S. military photo identification card

      -         A current official school identification card with photo.

      -         A student, high school or above, may present a school yearbook picture with name listed

       

      All identification must include a photo that clearly identifies the applicant. The identification must be current, legible and may not be torn, broken or mutilated. The dates of issue and expiration must be clearly legible. 



    • Hours:  8:30 am - 3:30 p.m.  Monday – Friday (excluding holidays)


    • Fees: Fees are payable by  CASH or CREDIT/DEBIT.
    •  Birth Certificate:    $34.00 

       Death Certificate:   $26.00

    • IMPORTANT: In cases where applicants provide or attest to false information, the individual who signs the application is the individual prosecuted in accordance with LA R.S. 40:61 a fine of not more than ten thousand dollars or imprisonment of not more than five years, or both shall be imposed on any person who willfully and knowingly makes any false statement on any application for a certified copy of a vital record.


ALL FEES PAID TO THE ASCENSION CLERK OF COURT ARE NON-REFUNDABLE.
